Submission Company Agrees to the Terms below:

1. All invoices are to be paid 30 days from the date of the invoice.
2. Claims arising from invoices must be made within seven working days.
3. By submitting this application, you authorize 3millID Corporation to do a credit check.
4. All 3millID Standard Terms and Conditions apply.
5. Submitting guarantor has the authority and is authorized by the Company to bind the Company with respect to all purchases from 3millID.
6. If Buyer determines they want to Wire Transfer funds for payment to 3millID, after receiving 3millID's email banking instructions, and immediately prior to sending any funds, Buyer will re-confirm 3millID's Bank Name, Routing Number and Account Number as further verification, live over the phone, with an Executive of 3millID. Buyer also agrees to not change the Wire Transfer Information without written AND Verbal communication over the phone with an Executive of 3millID. This second voice verification protects both parties from potential fraud.
